New Patient Registration

You can come into the surgery any time during opening hours and ask at reception for a new patient registration form.

Please note: if you are aged over 18 years we do require a form of photo ID as proof of identification and also proof of address prior to registering you. All new patients are asked to book a new patient check with the Practice Nurse.

You can either fill the 'New patient application form' out there and then at reception or you can take it away with you.

Alternatively you can download and print a 'New patient application form' here and take it completed to the surgery.

An appointment then needs to be booked with one of our Practice Nurses for a new patient check, this is just a general check for things like your blood pressure, weight etc.
